042 Are We Measuring the Right Thing?
One number can be useful. One number can also be misleading. Perhaps one of the most important lessons in health—and in science—is learning the difference.
When I was in elementary school, our class participated in the annual Presidential Physical Fitness Test.
I loved being active.
I enjoyed running, playing sports, and spending time outdoors. On most of the events I performed very well.
There was just one problem.
I couldn't do a pull-up.
Because of that single event, I didn't qualify for the fitness award.
One day my coach, Brad Bramhall, called me over.
He showed me a letter he had written to the people responsible for the program.
In that letter he explained that he believed the test wasn't fully measuring the overall fitness of some students. He used me as one example. He felt the award should recognize total physical ability rather than depending so heavily on one exercise.
They thanked him for writing.
They did not change the program.
Years later, after college, my father and I stopped to visit Coach Bramhall.
I thanked him.
I wanted him to know that I had never forgotten what he had done.
The award itself had long since become unimportant.
His belief in one student had not.
A Lesson Much Bigger Than Fitness
For many years I thought this story was simply about a thoughtful coach.
Later, after studying chemistry and working in analytical laboratories, I realized it was also about something much larger.
It was about measurement.
Good science depends on measurement.
But before measuring anything, we must first ask an even more important question.
Are we measuring the right thing?
A beautifully calibrated instrument can produce extremely accurate numbers.
If it measures the wrong variable, however, those accurate numbers may still lead to poor conclusions.
The same principle applies far beyond the laboratory.
Precision Is Not the Same as Understanding
Imagine a bathroom scale.
It may tell you that you weigh exactly 182.4 pounds.
The measurement might be extremely precise.
But what does it actually tell you?
It does not tell you:
- How much of that weight is muscle.
- How much is body fat.
- How strong you are.
- How healthy your heart is.
- How well you sleep.
- How active you are.
- How happy you feel.
The scale is not wrong.
It simply measures only one part of a much larger story.
The Body Is More Than One Number
Modern medicine has given us many valuable measurements.
Each one contributes useful information.
None tells the whole story by itself.
Body Weight
Useful?
Absolutely.
Complete?
No.
Two people may weigh exactly the same while having very different amounts of muscle, bone, and body fat.
Body Mass Index (BMI)
BMI has helped researchers study large populations for decades.
For an individual person, however, it has important limitations.
A muscular athlete may be classified as overweight.
An older adult with little muscle may have a "normal" BMI while struggling with weakness.
The number has value.
It simply should not be mistaken for the whole picture.
Blood Pressure
Blood pressure is one of the most useful measurements in medicine.
Even so, one unusually stressful afternoon can produce a different reading than several quiet mornings measured under similar conditions.
Patterns often tell us more than isolated measurements.
Blood Sugar
A single blood glucose measurement provides a snapshot.
Other tools, such as hemoglobin A1c or continuous glucose monitoring, provide additional perspectives.
Together they create a more complete understanding.
Cholesterol
Cholesterol is another excellent example.
Years ago many conversations focused almost entirely on one number.
Today we recognize that physicians often consider many factors together, including cholesterol fractions, triglycerides, blood pressure, blood sugar, family history, smoking status, activity level, and other health conditions.
The picture became larger.
The Pull-Up Lesson
Looking back, my coach was not arguing that pull-ups were unimportant.
He simply believed that one exercise should not determine the overall evaluation of an active student.
Imagine comparing two people.
One can perform many pull-ups but becomes exhausted after walking one mile.
The other cannot complete a pull-up but hikes mountains, bicycles regularly, gardens for hours, and has excellent endurance.
Who is more physically fit?
The answer depends on what we choose to measure.
Nature Rarely Measures Just One Thing
Your body certainly doesn't.
Your heart, lungs, muscles, brain, immune system, hormones, and metabolism are constantly communicating with one another.
Health is not one measurement.
It is a network.
When we focus too narrowly on one number, we risk overlooking the remarkable complexity of the human body.
Lessons from Analytical Chemistry
After college I studied analytical chemistry and later worked in scientific research.
One lesson became clear very quickly.
An instrument may be extraordinarily precise.
But before trusting the result, scientists ask important questions:
- Was the sample collected correctly?
- Was the instrument calibrated?
- Were contaminants present?
- Is this the right method?
- Does this measurement actually answer the question we are asking?
Those questions often matter as much as the number itself.
Experimental Design: Asking Better Questions
Another valuable lesson came from studying experimental design.
A well-designed experiment does far more than collect data.
It asks whether the question itself has been framed correctly.
Researchers might ask:
- Did we measure the most meaningful outcome?
- Was the study long enough?
- Did we study the right population?
- Were there hidden variables?
- Could another explanation fit the results?
- Are we confusing correlation with causation?
Good experiments produce answers.
Great experiments begin with better questions.
The Danger of Chasing One Number
Many of us are tempted to focus on a single measurement.
- The number on the bathroom scale.
- The cholesterol result.
- The number of calories.
- The grams of protein.
- The number of daily steps.
- The hours of sleep.
Each can be useful.
None should become the entire definition of health.
A person who walks every day, sleeps well, enjoys meaningful relationships, eats mostly whole foods, and remains curious throughout life may be healthier than someone with a "perfect" number who neglects the rest of life.
Looking for Patterns
One of the greatest strengths of science is recognizing patterns.
Rather than becoming attached to one isolated measurement, scientists look for relationships.
How do several measurements change together?
Do they support one another?
Does the patient's story fit the laboratory results?
Does the laboratory result fit the biology?
The more pieces that point in the same direction, the greater our confidence often becomes.
Health Is a Mosaic
Imagine creating a beautiful mosaic.
Each small tile contributes something important.
Remove one tile, and the picture still exists.
Judge the artwork by only one tile, however, and the beauty disappears.
Health works much the same way.
Weight.
Blood pressure.
Fitness.
Sleep.
Nutrition.
Stress.
Relationships.
Purpose.
Curiosity.
Each is one tile.
Together they create the picture.
What Coach Bramhall Really Taught Me
As a child, I thought Coach Bramhall was standing up for me.
He was.
Looking back, I think he was also teaching a lesson that reaches far beyond physical education.
He was reminding us that fairness begins by measuring the right things.
That lesson applies to classrooms.
It applies to medicine.
It applies to scientific research.
It applies to everyday life.
One thoughtful teacher helped one student understand something that would become even more meaningful decades later.

Numbers matter.
Measurements matter.
Scientific instruments matter.
But wisdom begins before the first measurement is ever taken.
It begins by asking thoughtful questions.
Perhaps that is why the best scientists, the best teachers, and the best physicians often share something in common.
They are curious enough to wonder whether the obvious measurement is really the one that matters most.
Sometimes it is.
Sometimes it isn't.
Learning the difference may be one of the most valuable skills we ever develop.
